Understanding All-On-4 Plus®
All-On-4 Plus® places four or more implants per arch at carefully calculated angles to make the most of available bone. The angled positioning of the rear implants is what allows this approach to work even where some bone loss has already occurred, often without the need for extensive grafting beforehand.
Treatment follows a structured four-step protocol: digital planning with 3D CBCT scanning and Digital Smile Design software, surgical preparation of the bone and gum tissue, placement of the implants, and then Immediate Final Teeth™ within 24 to 48 hours. Digital Planning and Facial Analysis
3D CBCT scanning, facial photographs, and Digital Smile Design software build a precise picture of your bone structure, bite, and facial proportions before surgery is planned. You can see a simulation of your new teeth before the procedure begins. (1 visit, approximately 90-120 minutes)
Surgical Reshaping of Bone and Gums
The implant sites are prepared by reshaping the jawbone and carefully contouring the gum tissue, creating a clean, stable, and hygienic base for your final teeth.
Strategic Implant Placement
Four or more implants are placed according to the positions mapped out in your digital plan, with angled placement used to optimise stability and minimise the need for grafting. (Full day appointment, approximately 3-4 hours)
Immediate Final Teeth™
Your Immediate Final Teeth™ are custom-designed to fit your gum contours, smile line, and bite, and are fitted within 24 to 48 hours of surgery. These are your final teeth, not a placeholder.
Ongoing Maintenance
Long-term implant health depends on regular professional maintenance. This isn't optional; it's what supports the best possible long-term outcome. (Every 3-6 months, ongoing)
Risks & Recovery
All-On-4 Plus® is a substantial surgical procedure and the risks and recovery process need to be understood clearly before you proceed.
Possible risks include:
- Swelling, bruising, and discomfort in the days following surgery, which is expected and manageable
- Some bleeding during the initial healing period
- Infection or delayed healing at one or more implant sites
- Implant failure if osseointegration doesn’t occur as expected
Recovery considerations:
- Most of the initial healing takes place over one to two weeks

What are Immediate Final Teeth™ and how are they different from temporaries?
Most full arch implant protocols involve a temporary bridge while the implants heal, followed by a second procedure to fit final teeth months later. Immediate Final Teeth™ are different. They’re custom-designed before surgery using Digital Smile Design and milled from high-performance materials, then fitted within 24 to 48 hours of your implant placement. The integration process continues in the months following, but your final teeth go in from the start.
Do I need bone grafting with All-On-4 Plus®?
Possibly not. Angled implant placement is specifically designed to work with the bone that’s available, which is how many patients avoid the grafting that other implant approaches require. Where bone volume or density falls below what’s needed for a stable result, or where grinding puts higher load on the implants, additional implants or targeted grafting may be recommended after your assessment.
What does long-term maintenance involve?
Every three to six months, your New Gisborne team will carry out professional cleaning around the implants and prosthetic components and check the health of the surrounding bone and tissue. Between visits, specific home care tools and techniques recommended for your prosthetic type keep everything clean day to day.
